Teamsters Local 456 Eligibility Requirements

TO ALL LOCAL 456 BENEFIT FUND PARTICIPANTS: Your employer must contribute on your behalf for each benefit made available through Westchester Teamsters Local 456.

Your employer maintains a collective bargaining agreement with the Teamsters Local 456 Union. Each contract specifies the benefits for which you are covered.

6 Month Rule Pre-Certification Mandatory Second Opinion and Biological Parent Rules

A member or a dependent's claim for benefits must be submitted to the Fund Office within six months from the date of service .

Pre-Certification is required for all surgeries, for both in-patient and ambulatory procedures Members and dependents are required to contact the Fund Office prior to obtaining these surgical procedures. Failure in doing so may result in the benefit being paid at 50% of the allowable charges, or the entire claim may be denied.

In the event that the surgical procedures are emergencies, the pre-certification and the second opinion will be waived, and allowable fees will be covered by the Fund.

The Local 456 Health and Welfare Fund covers a dependent child for benefits if the member is the biological parent of the dependent child or had completed adoption proceedings.
