The Teamsters Local 456 Health and Welfare Plan provides full coverage for almost all hospital services through an arrangement with Empire Blue Cross . Should an eligible member or dependent need hospitalization, all the participant has to do is to show a Teamsters Local 456 Health and Welfare Fund EMPIRE card and all covered hospital services (except for telephone and television) will be paid in full. You must pre-certify all surgeries and/or admissions through Empire via the phone number on the back of the card. (1-800-553-9603)
