EYE CARE

Teamsters Local 456 Health and Welfare Plan provides two types of coverage for healthy eye care. Participants may choose one of the benefits each year. Covered members may receive $150 for full service healthy eye care including the purchase of prescription glasses. Or, participants may use the services of the Fund's participating eye care centers. To use this benefit, covered members must obtain an “eye care voucher” by calling or dropping by the Fund Office. To receive a list of participating providers, please contact the Fund Office. Members who use the participating vision centers should not incur any out-of-pocket expenses for healthy eye care services. unless they choose a non-covered service. The maximum eye care benefit is $150 per family member, per year.
